Furosemide, commonly known by the brand name LASIX, is a potent loop diuretic used primarily for the treatment of fluid retention (edema) and hypertension. It produces a stronger diuretic effect than many thiazide type medications and is particularly useful when substantial removal of excess fluid is required.
Furosemide works primarily within the loop of Henle in the kidneys, where it inhibits the reabsorption of sodium and chloride. This increases the amount of sodium, chloride and water eliminated through the urine, reducing circulating fluid volume and helping relieve swelling associated with fluid accumulation. Its high diuretic potency is closely related to this site of action within the nephron.
Clinically, furosemide is used to manage edema associated with congestive heart failure, liver cirrhosis and kidney disease, including nephrotic syndrome. Reducing excess fluid can help decrease swelling in the legs and other tissues and may relieve symptoms associated with fluid overload in appropriately diagnosed patients.
Oral furosemide may also be used for high blood pressure (hypertension), either alone or in combination with other antihypertensive medications. Its blood pressure lowering effect is partly related to reductions in circulating fluid and sodium, although therapy must be individualized according to patient response.

Because furosemide is a potent diuretic, excessive treatment can cause significant water and electrolyte depletion. Potential abnormalities include low potassium, low sodium, dehydration and reduced blood pressure. Fluid balance, electrolyte levels and kidney function may therefore require monitoring during treatment, especially in patients using higher doses or other medications that influence blood pressure or electrolytes.
Symptoms associated with excessive diuresis or electrolyte imbalance can include dizziness, weakness, muscle cramps, thirst, confusion or abnormal heart rhythm. Patients with impaired kidney or liver function, cardiovascular disease or existing electrolyte abnormalities may require additional monitoring and individualized treatment.
Furosemide can also increase uric acid and blood glucose levels in susceptible individuals, which may be relevant for patients with gout or diabetes. Other potential adverse effects include gastrointestinal symptoms, dizziness and changes in blood pressure.
