Ursodiol, also known as ursodeoxycholic acid (UDCA), is a naturally occurring hydrophilic bile acid used primarily in the management of primary biliary cholangitis (PBC) and other selected hepatobiliary conditions. Ursodiol normally represents only a small proportion of the body’s natural bile acid pool, but oral treatment increases its concentration within bile.
In cholestatic liver disease, potentially damaging hydrophobic bile acids can accumulate within the liver and biliary system. Ursodiol gradually replaces and displaces some of these more hydrophobic bile acids with a less toxic, more hydrophilic bile acid, helping protect liver cells and bile duct epithelial cells from bile acid related injury.
Ursodiol has several complementary mechanisms of action. In addition to altering the composition of the bile acid pool, research and prescribing information describe cytoprotective effects on cholangiocytes, stimulation of bile secretion, inhibition of hepatocyte apoptosis, and immunomodulatory activity. These actions contribute to its established role in the long term management of PBC.
Ursodiol is also associated with cholesterol gallstone management in selected patients. Certain ursodiol formulations are indicated for dissolution of small, radiolucent, non calcified cholesterol gallstones when surgery is unsuitable or declined, and for prevention of gallstone formation during rapid weight loss. Ursodiol changes bile from a cholesterol precipitating environment toward one that is more favorable for cholesterol solubilization.

Liver function should be monitored during treatment. Current prescribing information recommends monitoring markers including ALT, AST, alkaline phosphatase, GGT and bilirubin, particularly during the early months of therapy and periodically thereafter. Ursodiol is contraindicated in patients with complete biliary obstruction.
Commonly reported adverse effects can include abdominal discomfort, abdominal pain, diarrhea, nausea, itching and rash. Bile acid sequestrants and aluminum containing antacids may reduce ursodiol absorption and can interfere with its effectiveness.
