Minoxidil is an oral systemic vasodilator originally developed for the treatment of severe or treatment resistant hypertension. Oral minoxidil reduces peripheral vascular resistance by relaxing vascular smooth muscle, with its pharmacological activity linked to opening ATP sensitive potassium channels. Increased hair growth was later recognized as a characteristic effect of systemic minoxidil, leading to extensive investigation of the compound for hair loss conditions.
Today, low dose oral minoxidil (LDOM) is commonly prescribed off label by dermatologists for androgenetic alopecia and other forms of hair loss. Published reviews describe oral minoxidil doses ranging from approximately 0.25mg to 5mg daily in hair loss treatment, with higher low dose regimens more commonly studied in men. Oral minoxidil is not specifically approved for hair loss, so this application remains off label.
The precise mechanism by which minoxidil stimulates hair growth has not been completely established. Its effects are believed to involve potassium channel activity around the hair follicle, changes in follicular signaling and prolongation of the anagen, or active growth, phase of the hair cycle. These effects can increase hair diameter and encourage continued growth in susceptible follicles.
Clinical research has specifically evaluated oral minoxidil 5mg in men with androgenetic alopecia. A prospective study reported increases in hair growth after 24 weeks of once daily 5mg oral minoxidil, supporting continued research and off label clinical use of this strength for male pattern hair loss.

Another well recognized effect of oral minoxidil is hypertrichosis, or increased hair growth in areas outside the scalp. This effect is dose related and has been frequently reported in studies evaluating 5mg oral minoxidil for androgenetic alopecia.
Oral minoxidil should therefore be used under the direction of a qualified healthcare professional, particularly in individuals with cardiovascular disease, abnormal blood pressure, kidney disease or other conditions that may increase the risks associated with systemic vasodilation.
