Raloxifene is a selective estrogen receptor modulator (SERM) that produces different estrogen like or estrogen blocking effects depending on the tissue involved. Rather than increasing estrogen levels throughout the body, raloxifene binds directly to estrogen receptors and selectively activates or inhibits estrogen regulated pathways.
In bone tissue, raloxifene acts primarily as an estrogen receptor agonist. Estrogen deficiency following menopause accelerates bone resorption and can progressively reduce bone mineral density. Raloxifene helps decrease bone resorption and overall bone turnover, supporting bone mineral density and reducing the incidence of vertebral fractures in postmenopausal women with osteoporosis.
Its tissue selective activity distinguishes raloxifene from conventional estrogen replacement therapy. While providing estrogen like activity in bone, clinical and preclinical evidence indicates that raloxifene has anti estrogenic activity in breast and uterine tissues and does not produce the same estrogen like stimulation of these tissues associated with systemic estrogen therapy.
Raloxifene has also been studied extensively for its effects on estrogen receptor positive breast tissue. In the United States, it is approved for reducing the risk of invasive breast cancer in certain postmenopausal women with osteoporosis or those considered at high risk. It is important to distinguish risk reduction from cancer treatment: raloxifene is not a treatment for existing invasive breast cancer and does not eliminate the possibility of developing breast cancer.
