Citalopram works by selectively inhibiting the reuptake of serotonin (5 hydroxytryptamine or 5 HT) at nerve terminals in the central nervous system. Unlike some older antidepressant medications, citalopram has relatively limited direct activity at many other neurotransmitter receptors. Citalopram has several important safety considerations. Antidepressants can increase the risk of suicidal thoughts or behaviors in children, adolescents, and young adults, particularly during the early stages of treatment or following changes in therapy. Patients should be appropriately monitored for worsening depression, unusual changes in behavior, or emergence of suicidal thoughts.
Citalopram can also cause dose dependent QT interval prolongation, which can increase the risk of potentially serious abnormal heart rhythms in susceptible individuals. Current prescribing information therefore recommends against doses above 40mg per day and identifies additional precautions for patients with certain cardiac conditions, electrolyte abnormalities, liver impairment, or medications that also prolong the QT interval.
Other potential adverse effects include nausea, dry mouth, sweating, fatigue, sleep disturbances and sexual dysfunction. Like other serotonergic medications, citalopram can also contribute to serotonin syndrome when combined with certain other medications that increase serotonin activity.
Citalopram should generally not be discontinued abruptly after regular use. Sudden discontinuation can produce symptoms such as dizziness, irritability, sensory disturbances, anxiety, headache, and sleep changes. Prescribing information recommends gradual dose reduction when treatment is discontinued whenever possible.
