Telmisartan is a prescription angiotensin II receptor blocker (ARB) used primarily for the treatment of hypertension (high blood pressure). It acts on the renin angiotensin aldosterone system (RAAS), one of the body’s major regulatory systems for blood pressure, vascular tone, sodium balance, and fluid retention.
Telmisartan selectively blocks the angiotensin II type 1 (AT1) receptor. Angiotensin II normally binds to these receptors and produces several cardiovascular effects, including constriction of blood vessels, stimulation of aldosterone release, increased sodium retention, and elevation of blood pressure. By preventing angiotensin II from activating AT1 receptors, telmisartan helps blood vessels relax and reduces blood pressure. Telmisartan has a very high selectivity for the AT1 receptor compared with the AT2 receptor. Its antihypertensive activity therefore results from blocking the vascular and aldosterone related actions of angiotensin II without directly inhibiting the angiotensin converting enzyme (ACE).
Unlike ACE inhibitors, ARBs such as telmisartan do not directly inhibit the breakdown of bradykinin. This pharmacological difference is one reason ARBs may be considered in some patients who cannot tolerate ACE inhibitors. Telmisartan is indicated for the treatment of hypertension, where lowering persistently elevated blood pressure helps reduce the risk of cardiovascular events such as stroke and myocardial infarction. Certain telmisartan formulations are also indicated for cardiovascular risk reduction in selected high risk patients who are unable to take ACE inhibitors.

The 40mg strength is an established telmisartan tablet formulation. Health Canada currently lists MICARDIS 40mg and several generic telmisartan 40mg tablets as marketed prescription medications in Canada.
Telmisartan has a relatively prolonged pharmacological effect, making it suitable for sustained blood pressure control when prescribed appropriately. For hypertension, current prescribing information recognizes 40mg as a standard starting strength, with treatment individualized according to blood pressure response and patient factors. One of the most important safety considerations with telmisartan is hyperkalemia, or elevated potassium levels. Because blockade of angiotensin II can reduce aldosterone activity, potassium retention may occur, particularly in people with kidney disease or those taking potassium supplements, potassium sparing diuretics, or other medications that increase potassium.
Telmisartan can also cause low blood pressure, dizziness, and changes in kidney function, particularly in patients who are dehydrated, sodium depleted, taking multiple blood pressure medications, or have certain forms of kidney disease. Kidney function and serum potassium may require monitoring during treatment. Non steroidal anti inflammatory drugs (NSAIDs) can reduce the antihypertensive effect of telmisartan and may increase the risk of impaired kidney function in susceptible patients. Combining medications that act on multiple parts of the renin angiotensin system can also increase the risk of hypotension, hyperkalemia, and kidney complications.
Telmisartan and other medications acting directly on the renin angiotensin system can cause serious fetal harm when used during pregnancy. Treatment should be discontinued promptly if pregnancy occurs and appropriate medical advice should be obtained.
