Eplerenone is a prescription mineralocorticoid receptor antagonist (MRA) used in cardiovascular medicine for the treatment of hypertension and in certain patients with heart failure with reduced ejection fraction (HFrEF). Health Canada currently lists marketed eplerenone 25mg tablets, including the brand INSPRA and generic formulations, as prescription medications in Canada.
Eplerenone works by selectively blocking the effects of aldosterone, a steroid hormone produced by the adrenal glands that regulates sodium, water, and potassium balance. Aldosterone normally binds to mineralocorticoid receptors in the kidneys and other tissues, promoting sodium and water retention while increasing potassium excretion.
By antagonizing the mineralocorticoid receptor, eplerenone reduces aldosterone mediated sodium retention and encourages the excretion of sodium and water. This can decrease circulating fluid volume and blood pressure while reducing the cardiovascular effects associated with excessive aldosterone signaling.
Unlike conventional potassium wasting diuretics, eplerenone is considered a potassium sparing medication. Its ability to reduce potassium excretion is clinically important but also means that excessive potassium accumulation, or hyperkalemia, is one of the principal risks associated with treatment.
Eplerenone has established use for high blood pressure, where lowering blood pressure can reduce long term cardiovascular risks including stroke and myocardial infarction. It is also indicated to improve survival in appropriate patients with symptomatic HFrEF following an acute myocardial infarction.
The mineralocorticoid receptor is present not only in the kidneys but also in cardiovascular tissues. Chronic excessive aldosterone signaling can contribute to sodium retention and cardiovascular remodeling, which helps explain the clinical role of mineralocorticoid receptor antagonists in selected patients with heart failure.

Patients should not routinely use potassium supplements or potassium containing salt substitutes while taking eplerenone unless specifically directed by a healthcare professional. Symptoms such as an irregular heartbeat, significant weakness, dizziness, or other concerning changes should be medically assessed.
Eplerenone is metabolized primarily through the CYP3A4 enzyme system, so medications that strongly inhibit CYP3A4 can substantially increase eplerenone exposure and the risk of adverse effects. Potential drug interactions should therefore be reviewed before treatment.
Other possible adverse effects can include dizziness, low blood pressure, increased creatinine, and changes in kidney function or electrolyte balance. Appropriate monitoring depends on the patient’s cardiovascular condition, kidney function, potassium levels, and other medications being used.
