Nolvadex is a brand name for tamoxifen citrate, a prescription selective estrogen receptor modulator (SERM). Tamoxifen interacts with estrogen receptors in a tissue selective manner and is particularly known for blocking estrogen activity in breast tissue. Each tablet provides 20mg of tamoxifen, with 50 tablets supplied per package.
Tamoxifen is used in the treatment of estrogen receptor positive breast cancer, including certain early and metastatic breast cancers. It is also used following treatment of ductal carcinoma in situ (DCIS) to reduce the risk of invasive breast cancer and may be prescribed to reduce breast cancer incidence in certain women at elevated risk.
Tamoxifen binds to estrogen receptors and changes how estrogen responsive genes are activated. In breast tissue it primarily acts as an estrogen antagonist, limiting estrogen driven signaling that can promote the growth of hormone sensitive breast cancer cells. Its effects differ between tissues, which is why it is classified as a selective estrogen receptor modulator rather than a pure estrogen blocker.
Tamoxifen primarily works by binding to estrogen receptors and altering estrogen signaling. Aromatase inhibitors such as anastrozole, letrozole, and exemestane instead reduce the body's production of estrogen by targeting the aromatase enzyme. Both approaches can reduce estrogen related signaling, but they act through fundamentally different mechanisms.
Tamoxifen has important risks that require medical supervision, including venous thromboembolism such as deep vein thrombosis and pulmonary embolism, stroke, and changes to the endometrium including an increased risk of endometrial cancer. Other adverse effects can include hot flashes, menstrual changes, and visual or ocular problems. The balance of benefits and risks depends on the patient's indication and individual medical history.
