Dutasteride is an oral 5 alpha reductase inhibitor that significantly reduces the conversion of testosterone into dihydrotestosterone (DHT). DHT is a potent androgen involved in prostate growth and the progressive miniaturization of androgen sensitive hair follicles associated with male pattern hair loss. Dutasteride 0.5mg is medically approved for the treatment of symptomatic benign prostatic hyperplasia (BPH) in men with an enlarged prostate.
Unlike finasteride, which primarily inhibits Type II 5 alpha reductase, dutasteride inhibits both Type I and Type II 5 alpha reductase enzymes. This broader enzyme inhibition results in substantial suppression of circulating DHT and reduces androgenic signaling within tissues such as the prostate.
In men with BPH, reducing DHT can gradually decrease prostate volume and improve urinary symptoms associated with prostate enlargement. Dutasteride is indicated to improve BPH symptoms, reduce the risk of acute urinary retention, and reduce the likelihood of requiring BPH related surgery.
DHT is also an important contributor to androgenetic alopecia (male pattern hair loss). Although hair loss treatment is not an approved indication for dutasteride in Canada, clinical studies have investigated 0.5mg dutasteride for androgenetic alopecia and demonstrated improvements in hair growth. Its use specifically for hair loss is therefore considered off label.

Because dutasteride lowers prostate specific antigen (PSA) concentrations, patients undergoing prostate screening should inform their healthcare provider that they are taking dutasteride so PSA results can be interpreted appropriately. Dutasteride containing products are not approved for the prevention of prostate cancer.
Potential adverse effects can include decreased libido, erectile dysfunction, ejaculation disorders, and breast tenderness or enlargement. As with other medications that substantially suppress DHT, sexual and reproductive effects should be discussed with a healthcare professional when clinically relevant.
Dutasteride can be absorbed through the skin. Individuals who are pregnant or may become pregnant should not handle leaking or damaged capsules, because inhibition of DHT can interfere with normal development of the external genitalia of a male fetus. Capsules should be swallowed whole and should not be opened, crushed, or chewed.
